OR I am a home-based food business selling food in keeping with the Health Protection and Promotion Act (HPPA) and the Food Premises Regulation. • I or my staff hold a valid Food Handlers Certificate and it will be on-site on the day of the event. Food Operational Terms(Required) I agree to the food operational terms.• I am able to operate in cold and/or rainy weather. • I agree that no alcohol of any sort will be sold or served during the event. • I understand that I must display all allergy warnings at my booth. • I agree to operate my food booth for the total duration of the event. • I agree to maintain my premises in a clean and orderly condition throughout the day. • I agree to remove food containers and litter and dispose of them after the event. • I understand I must follow safe food handling procedures (wash hands, clean utensils, clean surfaces, avoid cross-contamination, keep food at safe temperatures, wrap or cover until serving).InsuranceEach Vendor is required to have insurance for this event and submit a Certificate of Insurance to Downtown Carleton Place BEFORE the event. We’ve partnered with Duuo Insurance to offer Bridge St Summerfest vendors affordable short-term insurance coverage that meets our requirements. A link to purchase a policy will be sent with your acceptance. If you have insurance for your business, please provide a Certificate of Insurance naming the Town of Carleton Place and Downtown Carleton Place BIA as additional insured. Please wait until your application is accepted before buying insurance. Insurance through DUUO or a COI must be sent to use before July 4th. You will not be allowed to participate in the Bridge St Summerfest without providing a certificate of insurance. Insurance Provider(Required) I will purchase DUUO insurance (a link will be sent with my acceptance email) I have my own insurance policy and will email a Certificate of Insurance PDF to info@downtowncarletonplace.com after my application approval.
